For insurance agents and agencies
From the client call straight to the insurer's form.
Formyza reads the transcript and summary of your phone and Zoom calls, extracts the client's details, and fills each insurer's application in that insurer's own structure. You review, approve and send.

The process
Five steps, and only one of them is yours.
Call
Record as usual, by phone or Zoom. The transcript and summary are created automatically.
Client sheet
Every fact from the call is stored once, in one structure, with the quote and the minute it was said.
Form mapping
The sheet is translated into the form of the insurer you choose: its fields, wording and order.
Fill the gaps
Anything not said on the call goes to the client as a short WhatsApp questionnaire. Nothing is guessed.
Approve and sign
You go over the form, each field with its source, and approve it for sending.
Your step
One form per insurer
Every insurer has its own form. You keep one client sheet.
The same answer lands differently on each form. "I quit smoking three years ago" is "No" to a question about the last 24 months, and "Yes" to "Have you ever smoked?". Formyza keeps a spec for every form of every insurer and translates each fact into that form's exact question.
Think of a universal power adapter: one socket on your side, the right plug for each insurer. A new insurer or a new form version means adding a form spec, with no development. Versions are kept, and a changed form is flagged for review.
| Form | Question on the form | What gets written |
|---|---|---|
| Insurer A | Smoked in the last 24 months | No |
| Insurer B | Have you ever smoked? | Yes, quit 3 years ago |
| Insurer C | 4.2 Smoking habits | Former smoker, over 2 years |
Trust and privacy
A question nobody asked stays blank.
On a health declaration, a wrong answer is non-disclosure. So Formyza is built around one rule: it fills only what was said, and flags everything that was not.
No guessing on health questions
Silence on a call is not a "No". An unasked question is marked "Not asked" and sent to the client.
Every field has a source
Each value carries the quote and the minute in the call, so it is easy to check, correct and document.
The agent always approves
Nothing is sent to the insurer automatically. A form goes out only after review, approval and signature.
Designed for sensitive data
Encryption, per-agent permissions and a retention and deletion policy, in line with Israel's Privacy Protection Law.
By RemitRix
Formyza is built by RemitRix, which builds risk and data infrastructure for Israeli institutional investors: insurers, pension funds and investment houses. The same care for data, now on the agent's side.
Pilot
A pilot for selected agencies.
We work with a small number of agencies on real forms, one insurer and one product at a time. Pilot agencies get their forms set up, hands-on support and special terms.
- Setup of the insurers' forms you work with
- Hands-on support in the first weeks

FAQ
Questions agents ask.
Which insurers does it work with?
Any form that can be given a spec. In the pilot we start with the insurers and products you work with most.
Do I need a new transcription system?
No. Formyza works with recordings of phone and Zoom calls and creates the transcript and summary from them.
What happens when something was not said on the call?
The field is marked "Not asked" and sent to the client as a short questionnaire. Formyza never fills values by guessing.
Is the form sent to the insurer automatically?
No. The agent reviews, corrects and approves the form, and only then does it go out for signature and sending.
How much does it cost?
Pricing will be set together with the pilot agencies, based on the number of agents and case volume.
